The AzRHA is seeking candidates interested in serving a 3-year term on the Board for terms beginning August 1, 2007. The Board of Directors is responsible for conducting the business of the Association and for leading legislative action on the resolutions passed at the Arizona Rural Health Conference by AzRHA members during the membership meeting. To be placed on the ballot, nomination forms are due July 10, 2007.

AzRHA invites your nominations of outstanding colleagues, programs, and legislators. Nominations must be received no later than *June 29, 2007* to be considered. Please
submit all completed nomination forms no later than 5:00pm on June 29, 2007, to:

Award winners will be honored at the Rural Health Conference Awards
Luncheon on Wednesday, August 1, and highlighted in the Luncheon Program.

Awards will be given in four categories:
- Distinguished Martha Ortiz Volunteer
This award
honors and individual who has contributed extensive time, dedication, and commitment to
supporting a rural health program or project that is making an identifiable difference in the health care provided to people in Arizona's rural
and/or medically underserved communities. This award also considers and individual who has been a strong advocate in influencing and
shaping policies and programs that impact the provision of health care to residents of rural and/or medically underserved communities in
Arizona.
- Exceptional Rural Health Professional
This award
recognizes a direct service provider for leadership in bringing health services to rural populations. The Professional Award is meant to be inclusive of all health disciplines (e.g., dentistry, optometry, nursing, behavioral health, public health, etc.). Factors taken into consideration include providing outstanding care, involvement in the community, and lasting contributions to the health care
system. The Professional of the Year has made a significant contribution to rural health through distinguished personal leadership.
- Outstanding Rural Health Program
This award
recognizes a community, at the local or statewide level that involves one or more health professionals or
entities, that promotes or facilitates the development of rural health delivery systems. Factors considered include coordination of services, networking
and collaboration, innovation in development and implementation, and lasting impact of the program on populations served. This award honors excellence
in the provision of health services to rural and/or medically underserved people.
- Outstanding Arizona Legislator
This award recognizes an Arizona Legislator for her/his continuing leadership, dedication, commitment, and
support in collaborating with rural health constituencies to advance policy that increases access to quality, affordable health care for rural Arizonans, and
supports programs that are making a difference in the lives of residents living in Arizona rural and/or medically underserved areas. The outstanding
legislator demonstrates knowledge of the issues impacting health care services in rural and/or underserved areas.
