The Arizona Rural Health Plan 2005-2007 is produced by the Southwest Border Rural Health Research Center (SBRHRC), Rural Health Office (RHO), UA Zuckerman College of Public Health with support from the Arizona Rural Hospital Flexibility Program and the Arizona State Office of Rural Health (SORH) Program, both funded by the Office of Rural Health Policy, Health Resources and Services Administration, Department of Health and Human Services.
The Rural Health Office would like to thank the members of the statewide Arizona Rural Health Plan Advisory Committee for their contributions to the development of the Plan; those who provided feedback on the Plan drafts; and those who participated in the Delphi Study.
The Arizona Rural Health Plan has been approved by the Centers for Medicare and Medicaid Services (CMS) based on the requirements of the Rural Hospital Flexibility Program. (4/14/05)
